A subcontractor prequalification questionnaire helps you decide whether a company fits the proposed work before you award it. Ask about relevant experience, available capacity, and requirements that could prevent the sub from starting.

A short questionnaire to adapt

  1. What is your legal business name, and who handles contracts?
  2. Which parts of this scope will you perform yourself?
  3. Which parts will you subcontract further?
  4. What similar projects have you completed?
  5. Who can provide a reference for that work?
  6. Who will supervise this project?
  7. What crew and schedule can you commit to?
  8. Which required licenses do you hold?
  9. Can you meet the project’s insurance and bonding requirements?
  10. What assumptions or exclusions are in your proposal?

Review the answers against this job

A strong reference on a small renovation may not answer your questions about a complex occupied facility. Match the review to the scope, schedule, and consequences of a missed commitment.

For example, a sub may have suitable experience but no available foreman until two weeks after your start date. Resolve that before treating the proposal as ready for award.

Request supporting records selectively

Check applicable licenses at the issuing authority. Contact references and ask about comparable work. Have your risk team identify any additional safety or financial review the project needs.

Use a restricted process for sensitive financial information. A short intake questionnaire should not scatter those records across project inboxes.

Record the reviewer, outcome, and open conditions. Prequalification is an assessment for a proposed job; it does not replace reviewing current insurance or required documents before work begins.
